        <article-title>What Should We Measure When Someone Says Their Back Feels Hot?</article-title>
        <trans-title-group xml:lang="ko"><trans-title>등이 뜨겁다는 말에서 무엇을 측정해야 할까</trans-title></trans-title-group>

      <abstract><p>“My back feels hot” is a clear statement, but the object of study is not. Skin-surface temperature, spontaneous warmth, burning pain, itch-associated burning, and responses to imposed thermal stimuli may be different phenomena. We searched modern biomedical research, Korean scholarship, related expressions in East Asian medical texts, and clinical safety guidance. The search did not reveal one cause. It revealed that several observations had been folded into one symptom name while different fields measured only fragments of it. This reader edition reconnects the evidence of four source papers as one scientific essay.</p></abstract>

      <title>The sentence was short; the research question grew longer</title>
      <p>“My back feels hot.” The sentence is vivid enough to understand without explanation, yet its clarity dissolves when research begins. Does the skin feel hot to touch? Does heat seem to arise from within despite ordinary skin temperature? Is the experience mixed with burning, stinging, itch, or pain? Is it confined near one shoulder blade or spread across the back? Does it occur only at night or change after posture and movement? One sentence contains several possible observations [<xref ref-type="bibr" rid="R1">1</xref>].</p>
      <p>The first task was not to attach a disease name to the sentence. Nor was it to choose between believing the person and believing the thermometer. It was to preserve, in a researchable form, the possibility that sensation and measurement may differ. The question moved from “What disease is this?” to “What must be observed separately?”</p>
      
    </sec>
    <sec id="separate-objects">
      <title>Dividing one word—heat—into four objects</title>
      <p>The first object is an experience arising without an imposed stimulus. The original words—hot, warm, burning, cold yet burning—are retained alongside location, perceived depth, boundary, intensity, and duration. The second object is physical skin-surface temperature, for which site, room conditions, comparator, and time since symptom onset are part of the value. The third is the response to experimentally imposed warming or cooling. It may resemble a spontaneous episode, but cannot be assumed to be the same phenomenon [<xref ref-type="bibr" rid="R1">1</xref>,<xref ref-type="bibr" rid="R3">3</xref>].</p>
      <p>The fourth object is safety context. A thermal adjective alone does not determine risk. New weakness, the distribution of sensory loss, gait change, bladder or bowel change, cancer history, and rapid progression can alter the appropriate assessment pathway. This fourth object is not another thermal measurement; it is the boundary that prevents research phenotyping from replacing individual clinical judgment [<xref ref-type="bibr" rid="R4">4</xref>].</p>
      
    </sec>
    <sec id="literature-between-fields">
      <title>There was abundant literature, but the question fell between fields</title>
      <p>The literature was not empty. PubMed returned hundreds of records on back and trunk sensation, neuropathic pain language, pruritic sensory syndromes, quantitative sensory testing, skin temperature, and thermography. The Korea Citation Index and OASIS were searched separately for Korean and Sinitic expressions combining body region, heat, and burning. Historical searches followed strings such as 背熱, 背中熱, and 背部灼熱 only through routes where edition and context could be examined [<xref ref-type="bibr" rid="R2">2</xref>].</p>
      <p>The quantity of literature was not the quantity of evidence directly answering the question. Temperature studies often did not ask about spontaneous felt heat. Sensory tests measured thresholds to external stimuli but did not capture episodes arising in everyday life. Clinical studies of the back usually recruited people by pain or chronic itch. The target phenomenon appeared at the edges of several fields while remaining central to none [<xref ref-type="bibr" rid="R1">1</xref>,<xref ref-type="bibr" rid="R2">2</xref>].</p>
      
    </sec>
    <sec id="burning-clue">
      <title>What studies recording burning on the back could—and could not—tell us</title>
      <p>One of the nearest bodies of evidence concerned notalgia paraesthetica, a syndrome defined by chronic upper-back itch and altered sensation. In a 2020 series of 45 people, burning was recorded as a pain characteristic in 22. In a 2012 Brazilian–German cohort of 65, sensory quality was available for 40 German participants: burning was recorded in 19 and warmth in two [<xref ref-type="bibr" rid="R5">5</xref>,<xref ref-type="bibr" rid="R6">6</xref>].</p>
      <p>The numbers were intriguing but not an answer. Participants were selected for an itch-defined condition, not for recurrent localized back heat. Reports did not fully establish whether words were volunteered or offered, whether warmth was asked separately, or whether multiple qualities could be selected. These studies show that burning and warmth do appear in records of dorsal sensation. They do not show that the words denote the same sensation, how common either is among people reporting localized heat, or what causes it [<xref ref-type="bibr" rid="R1">1</xref>,<xref ref-type="bibr" rid="R5">5</xref>,<xref ref-type="bibr" rid="R6">6</xref>].</p>
      
    </sec>
    <sec id="measurement-clue">
      <title>Measurement did not end the problem; it made it more precise</title>
      <p>Thermography shows that real temperature differences can occur across the back. A 2022 pilot study imaged skin-surface temperature over the back and abdomen after unilateral trunk-muscle fatigue, observing side differences and changes over time. But participants were not recruited for spontaneous heat, and felt warmth or burning was not measured. Thermography is not therefore meaningless; rather, the surface change captured by a camera cannot be called the same thing as the event a person felt [<xref ref-type="bibr" rid="R7">7</xref>].</p>
      <p>Quantitative sensory testing provides another fragment. Cold and warm detection thresholds vary across the trunk, and studies have compared healthy volunteers with people living with postherpetic neuralgia. A 2020 thermal-sensitivity map compared eleven torso regions in 42 young healthy participants and reported regional, sex, and individual differences. The journal full text was not openly available, so methodological detail cannot responsibly extend beyond the abstract [<xref ref-type="bibr" rid="R8">8</xref>,<xref ref-type="bibr" rid="R9">9</xref>]. As measurement became more precise, site, timing, and comparator became part of the research question itself.</p>
      
    </sec>
    <sec id="old-text">
      <title>Was an old medical text describing the same phenomenon?</title>
      <p>The Donguibogam did not arrange heat as one quantity. Its “Fire” chapter separately presents palpated depth, regional distribution associated with viscera, differences between day and night, and heat of the palms, soles, and chest. This is not evidence validating modern physiology, but it is a textual fact: distribution, depth, and time were treated as different axes of observation [<xref ref-type="bibr" rid="R10">10</xref>].</p>
      <p>Under the heading 背熱, the Donguibogam cites the Yixue Rumen: “Heat of the back belongs to the lung. Because the lung resides in the upper burner, heat responds at the back.” This sentence cannot be translated into a modern lung diagnosis or prescription. Nor can one sentence determine whether the heat was felt by the patient or confirmed by touch. The classical text therefore serves not as an answer key but as a comparison that broadens the question. It is notable that location, depth, and time were already editorial units in an older medical text when modern research is again trying to separate them. Notability, however, is not equivalence [<xref ref-type="bibr" rid="R10">10</xref>].</p>
      
    </sec>
    <sec id="negative-space">
      <title>When not finding something becomes an honest result</title>
      <p>Korean and East Asian sources were searched together, yet no verified genealogy was found connecting the modern experience of recurrent localized subjective back heat to historical wording. Some databases could not be searched as planned because their public interfaces were inaccessible. Several papers could be read only at abstract level because no lawful open full text was located. This does not prove that relevant research does not exist. It marks where the search reached and where claims must stop [<xref ref-type="bibr" rid="R2">2</xref>].</p>
      <p>The boundary is a limitation and also a map for the next study. The need is not merely for more adjacent literature. A future study must first preserve the participant’s words and body map at the moment spontaneous heat occurs, record concurrent skin temperature, and keep later evoked sensory testing as a separate variable. This is where a gap in literature becomes a measurement design [<xref ref-type="bibr" rid="R3">3</xref>].</p>
      
    </sec>
    <sec id="safety">
      <title>Failure to explain is not permission to reassure</title>
      <p>Refusing to assign one cause does not make every case trivial. In the NICE guidelines on suspected neurological conditions and on spinal metastases and metastatic spinal cord compression, “heat” or “burning” was not used as a stand-alone decision criterion. Instead, combinations of cancer history, progressive pain, weakness, gait change, sensory distribution, and bladder or bowel change altered urgency [<xref ref-type="bibr" rid="R4">4</xref>,<xref ref-type="bibr" rid="R11">11</xref>,<xref ref-type="bibr" rid="R12">12</xref>].</p>
      <p>These guidelines belong to specified populations and a UK healthcare setting; they cannot simply be converted into individualized Korean clinical instructions. They nevertheless contribute one important research principle: detailed phenotyping and continued assessment for conditions that must not be missed have to coexist. Safety judgment must not be absorbed into an interesting phenotype.</p>
      
    </sec>
    <sec id="next-observation">
      <title>What emerged was not a diagnosis, but the next observation</title>
      <p>The most concrete output of the search is a prospective observational protocol. Participants first describe the sensation in their own words and draw its boundary on a body map. Investigators decide which expressions qualify before seeing temperature or sensory-test results. Event time, a symptom-free comparison time, and a standardized visit are separated. Sensory quality and location, skin-surface temperature, evoked cold and warm perception, functional impact, and contexts such as posture, sleep, meals, and activity are recorded together without substituting for one another [<xref ref-type="bibr" rid="R3">3</xref>].</p>
      <p>Even if implemented, the first question will not be “Which disease did we discover?” It will be whether episodes can be captured in time, whether observations repeat within a person, whether sensation and temperature actually move together, and whether the burden of measurement is tolerable. If observations do not reproduce or burden overwhelms value, the design must be reduced or abandoned. Greater explanatory detail must arrive with greater capacity for disconfirmation.</p>
      
    </sec>
    <sec id="current-view">
      <title>What is clearer, what has weakened, and what remains unknown</title>
      <p>Three points are now relatively clear. First, felt heat, skin temperature, and evoked thermoception should be recorded separately. Second, neither “burning” nor “warmth” alone determines a neuropathic mechanism or specific disease. Third, region-based expressions in older medical texts do not diagnose a modern symptom, but may prompt modern research to recover overlooked axes of observation [<xref ref-type="bibr" rid="R1">1</xref>,<xref ref-type="bibr" rid="R2">2</xref>,<xref ref-type="bibr" rid="R3">3</xref>,<xref ref-type="bibr" rid="R4">4</xref>].</p>
      <p>Some ideas have weakened. The expectation that one thermographic image will reveal cause, the shortcut from burning to small-fibre dysfunction, and the reading of the Donguibogam’s “heat on the back” as a modern independent disease all exceed current evidence. Much remains unknown: how often felt episodes agree with concurrent skin temperature, whether patterns repeat within a person, what sensory testing adds, and whether Korean expressions reliably distinguish different experiences have not been directly studied.</p>
      <p>The significance of this work is therefore not a new explanation for back heat. It lies in unfolding a familiar phrase so that we can see what different eras and disciplines observed—and what each passed over. A good research question does not always narrow immediately toward an answer. Sometimes it separates several worlds folded into one word and thereby reveals what must finally be asked.</p>
